This meeting contained a presentation of the CoRe-Net project as well as a common discussion about value-based health care and about performance responsibility and process responsibility in health and social care.
Neben der Vorstellung des Projektes CoRe-Net beinhaltete das Treffen eine gemeinsame Diskussion über wertorientierte Versorgung (value-based healthcare) und über Ergebnis- und Prozessverantwortung in der Versorgung.
The goal is to use the structures of CoRe-Net for analyzing trajectories, potential barriers, and quality of care of patients with CHD with regard to the detection and treatment of mental disorders.
Ziel ist es, unter Einsatz der Strukturen des CoRe-Nets, Behandlungsverläufe und potenzielle Barrieren sowie die Versorgungsqualität von Patienten mit KHK im Hinblick auf die Erkennung und Behandlung potentieller psychischer Erkrankungen zu analysieren.
CoRe-Net currently includes three subprojects: LYOL-C, MenDis-CHD, and OrgValue. These projects, which examine health care across different care organizations for specific patient groups in Cologne, also aim to identify organizational characteristics hindering or facilitating the implementation of patient-centered care.
Aktuell werden im Rahmen von CoRe-Net drei Projekte (LYOL-C, MenDis-CHD und OrgValue) durchgeführt. Diese untersuchen die sektorenübergreifende Versorgung spezifischer Patientengruppen in Köln. Des Weiteren möchten wir Faktoren identifizieren, die eine patientenzentrierte Versorgung fördern bzw. hemmen.
Along with CoRe-Net, the Cologne Research and Development Network, the ZVFK supports the close ties with general practice, and thus contributes to participatory health services research.
